Overview:
\n Our well-established home health organization in Denver is seeking a Registered Nurse (RN) to provide comprehensive, skilled nursing care to adult patients in their homes. This role focuses on safe hospital-to-home transitions, individualized care planning, and long-term chronic disease management to improve patient outcomes and reduce rehospitalizations. New graduates and experienced RNs are both encouraged to apply.

Position Offers:

• Schedule: Monday–Friday, 8:00 AM–5:00 PM, with no on-call requirements
\n
• Competitive pay: $45–55 per hour (DOE)
\n
• Malpractice coverage provided
\n

Key Responsibilities:

\n
• Perform admissions, discharges, and re-evaluations for home health patients
\n
• Provide wound care and dressing changes
\n
• Manage patient medications
\n
• Conduct thorough patient assessments and develop individualized care plans
\n
• Support safe transitions from hospital or clinic to home
\n
• Coordinate with an interdisciplinary team to support long-term management of chronic conditions
\n
• Maintain consistent follow-through to improve patient outcomes and reduce rehospitalizations
\n
• Serve as a key clinical resource for patients and families
\n

Qualifications:

\n
• Current Registered Nurse (RN) license in Colorado, in good standing
\n
• Knowledge of OASIS preferred; training provided
\n
• Strong clinical assessment and care planning skills
\n
• Ability to work independently in a home health setting
\n
