Overview:
\n Our growing Home Health team is seeking an experienced Registered Nurse (RN) to support our patients within the Dallas area. This role involves managing patient cases, providing comprehensive skilled nursing care, and care coordination.

Position Offers:

\n
• Schedule: Monday–Friday, 8:00 AM–5:00 PM\n
  \n
  • Rotating weekend coverage every other weekend preferred, with two weekdays off during the rotation
  \n
\n
• Competitive pay: $43–$48/hour (DOE)
\n

Key Responsibilities:

\n
• Provide skilled nursing care within both facility and home-based settings
\n
• Complete Start of Care, Recertification, Follow-Up, Resumption of Care, and Discharge visits
\n
• Perform comprehensive patient assessments, including Oasis assessments, and coordinate individualized care plans
\n
• Manage skilled nursing needs including PICC line care, lab draws, wound care, wound vac management, and catheter care
\n
• Ensure timely, accurate documentation in the EMR system
\n
• Collaborate with providers, caregivers, and interdisciplinary teams to support positive patient outcomes
\n
• Provide high-quality care to a primarily geriatric patient population
\n

Qualifications:

\n
• Active Registered Nurse (RN) license in Texas
\n
• Previous Home Health experience required, with EMR proficiency
\n
• Oasis assessment experience required
\n
• Current TB clearance, valid driver's license, and BLS certification required
\n
• Strong clinical experience with wound care, PICC lines, lab draws, wound vacs (certification preferred), and catheter care
\n
