Family Medicine Family Medicine Physician (Adult Primary Care)

Position Overview

We are seeking a compassionate, experienced, and patient-focused Family Medicine Physician to join a well-established outpatient primary care practice in San Francisco, California. This is an excellent opportunity for a physician who is passionate about delivering comprehensive, evidence-based care to adult patients in a collaborative and patient-centered environment.

The ideal candidate is an experienced clinician with strong diagnostic skills, a commitment to preventive care, and the ability to build lasting relationships with patients while working alongside a multidisciplinary healthcare team.


Key Responsibilities

  • Provide comprehensive outpatient primary care services to adult patients.

  • Perform preventive health screenings, annual wellness visits, and routine physical examinations.

  • Diagnose and treat acute and chronic medical conditions.

  • Develop individualized treatment plans focused on improving long-term patient outcomes.

  • Manage chronic diseases such as hypertension, diabetes, hyperlipidemia, asthma, and other common adult health conditions.

  • Educate patients on disease prevention, healthy lifestyle choices, and wellness strategies.

  • Order and interpret laboratory tests, imaging studies, and other diagnostic procedures.

  • Coordinate referrals to specialists and collaborate with interdisciplinary healthcare providers to ensure continuity of care.

  • Maintain accurate and timely documentation within the Electronic Medical Record (EMR) system.

  • Participate in quality improvement initiatives and patient safety programs.

  • Mentor and support clinical staff while promoting a collaborative team environment.

  • Ensure compliance with federal, state, and organizational policies and clinical standards.


Qualifications

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school.

  • Board Certified or Board Eligible in Family Medicine.

  • Active or eligible for an unrestricted California medical license.

  • DEA registration (or ability to obtain prior to employment).

  • BLS certification (ACLS preferred if applicable).

  • Minimum 5 years of outpatient primary care experience preferred.

  • Strong clinical assessment and diagnostic skills.

  • Excellent communication, interpersonal, and patient education abilities.

  • Proficiency with Electronic Health Record (EHR/EMR) systems.

  • Demonstrated commitment to providing compassionate, high-quality, patient-centered care.


Preferred Qualifications

  • Experience caring for adult patient populations in an outpatient setting.

  • Leadership experience in clinical operations, physician mentorship, or quality improvement initiatives.

  • Experience working within multidisciplinary healthcare teams.

  • Strong commitment to preventive medicine and evidence-based clinical practice.
