Per Diem Physician - Post Acute & Senior Care Services (Cumberland)

Our organization is on the lookout for Per Diem physicians specializing in adult and senior care to enhance our expanding post-acute care team in the Cumberland area. This role is vital in delivering exceptional and compassionate care to our elderly patients. The post-acute care team operates across various skilled nursing facilities throughout the county.

We encourage applications from primary care physicians, hospitalists, and those with geriatric training.

Position Overview:

• Delivers direct patient care across a range of facilities, including skilled nursing, rehabilitation, personal care/assisted living, independent living, and clinics.
• Engages with facility leadership, hospital discharge teams, care management, and other specialties to ensure high-quality care aligned with shared objectives.
• Post-acute care encompasses various healthcare fields, from general primary care and internal medicine to geriatrics and acute rehabilitation services. Our clinicians collaborate with multiple specialists, enhancing their knowledge and skill sets.
• Works alongside and supervises Advanced Practice Providers.
• Utilizes EPIC EMR, online diagnostics, including PACS imaging and lab results.
• Conducts clinical rounds Monday through Friday, with appealing daily schedules and flexible models. Overnight and weekend on-call duties are limited to phone coverage, shared equitably among partners and APPs, with favorable ratios.

Professional Qualifications:

• Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
• A team player capable of delivering personalized patient care and demonstrating positive clinical outcomes.

• Eligible for licensure in Pennsylvania.
