Job Description
WCS Healthcare Partners is seeking a Licensed Practical Nurse (LPN) to provide comprehensive practical nursing care within a state-operated Veterans Long Term Care facility in Erie, Pennsylvania, serving honorably discharged Veterans with chronic medical conditions, physical disabilities, cognitive impairment, behavioral health needs, and age-related care requirements.
\nResponsibilities

- Provide practical nursing care and treatment for Veteran residents with medical, physical, cognitive, and behavioral health needs in a long-term care setting. \n
- Implement individualized nursing care plans and perform direct resident care in coordination with the interdisciplinary team. \n
- Prepare, administer, and accurately document medications and treatments prescribed by physicians. \n
- Observe, assess, and report changes in residents' physical, cognitive, and emotional conditions. \n
- Take and record vital signs, collect specimens, and maintain accurate clinical documentation and progress notes. \n
- Participate in Veteran-centered rehabilitation, restorative, and habilitation programs, including assistance with activities of daily living and therapeutic activities. \n
- May serve in a charge role for a unit or care area, supervising unlicensed personnel and supporting continuity of care. \n
- Perform nursing duties in accordance with federal/state regulations, facility policies, Veterans' care standards, and LPN scope of practice. \n
- Collaborate with RNs, physicians, and interdisciplinary team members to support resident care planning and outcomes. \n
- Respond to changes in resident condition and escalate clinical concerns to appropriate licensed personnel. \n
- Maintain compliance with infection control, safety protocols, and quality care standards. \n
- Participate in care planning meetings, interdisciplinary reviews, and ongoing resident evaluations. \n
Requirements
\n- \n
- Current Licensed Practical Nurse (LPN) license issued by the Pennsylvania State Board of Nursing, or a valid non-renewable temporary practice permit. \n
- 2 years of experience preferred. \n
- AHA BLS certification. \n
