Your Responsibilities

Provide Exceptional Patient Care

• Deliver direct physical therapy services under the supervision of a licensed Physical Therapist
• Adhere to personalized care plans to assist patients in alleviating pain, enhancing mobility, and restoring functionality
• Guide patients through therapeutic exercises, gait training, balance activities, and functional mobility
• Support patients with transfers, strength training, and daily living activities
• Employ therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as needed
• Educate patients and their caregivers on exercises, safety protocols, and the use of assistive devices

Track Progress & Communicate Effectively

• Monitor and report on patient progress, treatment responses, and any changes in their condition
• Maintain precise and timely documentation in the electronic medical record (EMR)
• Engage in clear communication with the Physical Therapist and interdisciplinary care team
• Participate in case discussions and care coordination initiatives

Collaborate & Contribute

• Work alongside nurses, therapists, and other healthcare professionals to ensure cohesive care
• Engage in team meetings, training sessions, and ongoing educational opportunities
• Adhere to clinical standards and contribute to quality improvement initiatives

Your Qualifications

• Graduate of an accredited Physical Therapy Assistant (PTA) program
• Current PTA license or certification as mandated by state regulations
• Preferred experience of 1+ year (home health experience is a plus)
• Current CPR certification
• Valid driver’s license, dependable transportation, and auto insurance
