What You’ll Do

Provide Patient-Centered Care

  • Assess and evaluate patients’ functional abilities, mobility, strength, and rehabilitation potential
  • Create and execute personalized care plans in partnership with physicians and the care team
  • Administer hands-on therapy to enhance mobility, strength, balance, and independence
  • Educate patients and their caregivers on treatment protocols, exercises, and safety measures

Drive Outcomes & Recovery

  • Track patient progress and modify treatment plans as necessary
  • Facilitate safe transitions of care and assist in discharge planning
  • Identify additional patient needs and collaborate with interdisciplinary teams

Collaborate & Communicate

  • Work closely with nurses, therapists, and physicians to ensure coordinated care
  • Accurately document patient care and progress in the EMR system in a timely manner
  • Engage in case conferences and team meetings

Lead & Support

  • Oversee Physical Therapist Assistants (PTAs) in compliance with state regulations
  • Offer guidance and education to caregivers and support staff

What You Bring

  • Graduate from an accredited Physical Therapy program (DPT or Master’s)
  • Active PT license in the state of practice
  • At least 1–2 years of experience (home health experience preferred)
  • Strong clinical assessment and critical thinking abilities
  • Excellent communication and patient education skills
  • Current CPR certification
  • Valid driver’s license, dependable transportation, and auto insurance
