As a Master’s Social Worker (MSW) in Hospice , you will be pivotal in assisting patients and families as they navigate the emotional, social, practical, and financial hurdles associated with terminal illnesses.

Evaluate the psychosocial needs of patients and families concerning terminal illness, end-of-life care, and their home environment.
Assess family dynamics, caregiver capabilities, communication styles, coping mechanisms, and potential psychosocial risks.
Offer counseling, emotional support, and crisis intervention to patients and caregivers as necessary.
Assist patients and families in managing stress, anticipatory grief, evolving family roles, and end-of-life issues.
Acknowledge and support cultural needs, traditions, values, and preferences that may affect care.
Educate and assist patients and families with advance directives and healthcare decision-making.
Help address practical needs, including financial, legal, funeral, and end-of-life planning.
Connect patients and families with community resources and support services.
Act as a liaison between patients, caregivers, community agencies, and other available resources.
Establish and maintain relationships with public and private community organizations that can assist patient and family needs.
Engage in the development and updating of personalized care plans with the patient, family, and interdisciplinary team.
Attend regularly scheduled interdisciplinary meetings and provide psychosocial insights for care planning.
Collaborate with physicians, nurses, chaplains, aides, and other team members to deliver holistic end-of-life care.
Help the care team understand significant social, emotional, cultural, and family factors affecting the patient’s experience.
Advocate for the needs, preferences, dignity, and quality of life of patients and families throughout the hospice journey.
Ensure accurate and timely clinical documentation, assessments, and progress notes.
Communicate changes in psychosocial needs, risks, and concerns to the relevant interdisciplinary team members.
Maintain effective communication with patients, caregivers, community partners, and the hospice team.
Participate in quality assessment and performance improvement initiatives.
Adhere to professional, ethical, and regulatory standards for hospice social work.

Master’s degree in Social Work (MSW).
Compliance with applicable hospice personnel qualifications and state regulations.
At least 1 year of MSW experience in a hospice environment.
Familiarity with hospice philosophy and issues surrounding death, dying, grief, and end-of-life care.
Ability to communicate tactfully and compassionately with patients, families, caregivers, community partners, and the care team.
Valid driver’s license, dependable transportation, and automobile insurance for home visits.
