Job Description
As a therapist within our organization, you will report to the Home Health Branch Administration and play a crucial role in assisting patients to regain their strength, mobility, and independence—all from the comfort of their own homes. By providing tailored care that emphasizes rehabilitation and functional enhancement, you will empower individuals to overcome physical challenges, confidently engage in daily activities, and improve their overall quality of life.
Key Responsibilities- Evaluate and screen patients' daily living and work-related skills, creating therapeutic retraining programs with measurable goals.
- Administer and interpret diagnostic and prognostic tests to assess patients' functional levels in their homes or other environments, aiding physicians in their evaluations.
- Collaborate with the patient's physician and other healthcare team members to develop or revise the Plan of Care Treatment.
- Deliver therapeutic treatments and guidance to patients as per physician directives to enhance strength, coordination, range of motion, and overall function, or to teach compensatory strategies.
- Review and adjust retraining programs in alignment with each patient's behavioral goals and the overarching rehabilitation program guidelines.
- Consult with vocational team members, as necessary, to create and implement vocationally focused plans that meet the needs and abilities of patients.
- Communicate patient status and progress to the physician, rehabilitation staff, Clinical Manager, and family members.
- Engage in care coordination and discharge planning activities, serving as a resource for other healthcare professionals to address patient needs.
- Design community reintegration activities to support clients in their physical recovery and psychological adjustment, coordinating with the interdisciplinary team.
- Recommend or create specialized adaptive equipment to enhance clients' home or work environments and facilitate their participation in rehabilitation and community activities.
- Train staff and family members to implement the retraining program, covering skills such as dressing, feeding, grooming, and hygiene, and participate in educational meetings with the team and family.
- Supervise, instruct, and assess the performance of Occupational Therapy Assistants and Home Health Aides in delivering occupational therapy services.
- Document patient care observations, interventions, and evaluations accurately and comprehensively on the day services are provided. Ensure that interim verbal orders from physicians are documented and executed properly. Submit evaluations, treatment plans, and discharge summaries to supervisors and care management staff.
Required:
- Current and unrestricted Occupational Therapy licensure.
- Current CPR certification.
- Valid driver's license, auto insurance, and reliable transportation.
Preferred:
- At least six months of experience in occupational therapy.
- Experience in home health settings.
Pay Structure: $54.00 - $76.00 per visit/unit.
Annual Salary: The estimated salary range for full-time employment (40 hours per week) is between $85,400 and $117,500, depending on geographic location and individual qualifications, including skills, experience, education, and certifications.
Additional InformationTB Screening: This position involves direct patient interaction and is part of our Tuberculosis (TB) screening program. If selected, you will need to undergo TB screening.
Driving Requirements: This role is included in our driver safety program, necessitating a valid state driver's license and personal vehicle liability insurance. You must maintain insurance that meets or exceeds your state's minimum requirements.
