The Sub-Acute and Long-Term Care sector is rapidly growing, offering excellent opportunities for work-life balance, with no on-call duties or weekend shifts! We are currently looking for a Full-Time provider (40 hours weekly) for our facility in Hampton, NH. This role is open to either a nurse practitioner or a physician assistant, and we encourage new graduates to apply.
The annual salary for this position ranges from $112,320 to $128,960, along with an incentive compensation bonus program that has no cap.

Key ResponsibilitiesThe Advanced Practice Provider will report directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. This role involves collaborating on-site with the Center Leadership Team to meet specific center goals and priorities. Additionally, the Advanced Practice Provider will work closely with the Medical Director and/or Attending Physician to deliver patient-specific medical care.
Responsibilities include:
- Providing primary and urgent care services to residents of the nursing center.
- Collaborating with other medical providers within the center.
- Working alongside Center Leadership to fulfill our mission of enhancing lives through high-quality healthcare and compassionate service.
The ideal candidate for the Advanced Practice Provider role must have graduated from an accredited School of Nursing and hold a valid registered nurse license. They should be certified as a Nurse Practitioner, having completed an accredited Nurse Practitioner Program with a Master's Degree in Nursing, focusing on Older Adult and/or Family Nurse Practitioner (FNP) specialties. Other concentrations may be considered based on experience and organizational needs.
For Physician Assistants, graduation from an ARC-PA accredited program is required, along with passing the PA National Certifying Exam (PANCE) to obtain the PAC designation. State licensure and NCCPA certification are also mandatory.
A minimum of two years of experience working with adult or geriatric populations is preferred.
In states where practice regulations permit, APRNs must hold an unrestricted professional license and DEA for each state of practice. Similarly, PAs must have an unrestricted professional license and DEA for their respective states.

Salary RangeThe posted salary range for this position is between $112,320 and $128,960 per year.
