Job Summary
We are seeking a compassionate and motivated Licensed Practical Nurse (LPN) to join a collaborative geriatric/senior primary care team focused on providing high-quality care to senior patients. The LPN will support the Primary Care Providers (PCP) with day-to-day clinical operations, assist with patient assessments and routine testing, and help ensure an exceptional experience for every patient.
The ideal candidate has experience working with geriatric/senior patients in a primary care or outpatient setting and is passionate about building trusting relationships with patients and their families.
Key Responsibilities
- Greet, room, and prepare geriatric and senior patients for primary care appointments.
- Conduct patient intake, including obtaining and documenting vital signs, medical history, and relevant health information.
- Perform routine screenings, examinations, and diagnostic testing as directed.
- Closely monitor senior patients and communicate changes in condition, concerns, or needs to the PCP, specialists, and appropriate clinical staff.
- Build strong relationships with patients while providing compassionate, respectful, and patient-centered care.
- Manage patient phone messages, return calls, route calls appropriately, and gather information for providers.
- Perform venipuncture and IV starts as permitted by state regulations.
- Provide basic clinical care, including dressing changes, bandaging, suture removal, and other first-aid procedures under provider supervision.
- Utilize CPR skills when necessary.
- Maintain accurate, timely, legal, and ethical patient documentation.
- Follow OSHA and all applicable safety and clinical standards.
- Provide health coaching to assigned senior patients to encourage healthy lifestyle choices and improved health outcomes.
- Follow up with coached patients through scheduled calls as directed.
- Collaborate effectively with PCPs, clinical staff, patients, family members, insurance carriers, and other healthcare partners.
Schedule: Monday-Friday 8a-5p
Salary Range: $54,000k-$66,560 (Based On Experience) + Internal Full Time Benefits

Required Qualifications
- High school diploma or GED required.
- Graduation from a nationally accredited Licensed Practical Nurse program.
- Current, active LPN license in the state of employment.
- Minimum of 1 year of LPN experience, preferably in geriatric primary care, senior care, or an outpatient clinic settings.
- Current BLS for Healthcare Providers certification.
- Strong communication, customer service, and interpersonal skills.
- High level of emotional intelligence, integrity, and professionalism.
- Excellent critical-thinking, decision-making, and problem-solving abilities.
- Ability to work independently while contributing effectively to a collaborative care team.
- Strong organizational skills and attention to detail.
- Ability to work effectively with diverse patients, particularly older adults and senior populations.
- Proficiency with Microsoft Office, including Word, Excel, PowerPoint, and Outlook.
- Ability to travel occasionally for company meetings.
Preferred Qualifications
- Experience working with geriatric/senior patients is strongly preferred.
- Experience in primary care, family medicine, internal medicine, or senior-focused healthcare.
- Experience with eClinicalWorks (eCW) or a similar electronic health record (EHR).
- Experience working with claims, quality reports, or analytical software.
- Strong analytical, strategic-thinking, and creative problem-solving skills.
