Minimum Qualifications

• Associate degree in Physical Therapy from an accredited institution.
• Active PTA license in Oregon is mandatory to commence the assignment.
• Prior home health experience in Oregon is essential.
• BLS/CPR certification is required (AHA or ARC accepted).
• Experience with patient follow-up visits and home health documentation is necessary.
• A valid driver’s license and the ability to travel between patient residences in Multnomah and Clackamas counties are required. Mileage reimbursement will be provided for travel between visits.
• Preferred: Familiarity with HCHB (Homecare HomeBase) electronic documentation.
