The Medical Social Worker, under the supervision of the Clinical Manager, plays a vital role in the interdisciplinary care provided to patients receiving home health services. Your primary responsibility will be to assess and create a tailored care plan that addresses the emotional and social needs of each patient. Additionally, you will oversee and guide the Social Worker Assistant involved in the patient's care plan. This position requires adherence to our organization's policies, healthcare standards, and relevant laws and regulations.

Key Responsibilities

• Evaluate the patient's emotional and social condition in relation to their illness or injury, including their care needs and responses to treatment.
• Examine the interplay between the patient's medical and nursing requirements within their home environment, financial resources, and community support options.
• Take appropriate actions to access community resources that can help address barriers to the patient's recovery.
• Engage in communication with patients, their families, and colleagues within the organization.
• Continuously assess the needs of patients and families, focusing on their understanding of the treatment process and coping with emotional and social challenges.
• Collaborate as part of the interdisciplinary care team to coordinate care and serve as a resource for other health professionals in identifying and addressing patient needs.
• Monitor the performance of the Social Work Assistant (BSW) to ensure compliance with company, governmental, and professional standards in delivering medical social services.
• Maintain and submit necessary documentation as required, including case conferences, patient and physician community contacts, visit reports, progress notes, and collaboration with other healthcare disciplines to ensure optimal patient care.

Qualifications and Requirements

• A minimum of one year of experience as a social worker in a healthcare environment, including home health or hospice settings.
• Licensure as a Social Worker in the state of practice, if mandated by state regulations.
• A valid driver's license, auto insurance, and dependable transportation.
• Current CPR certification.
• Experience in discharge planning and accessing community resources (housing, shelter, funeral arrangements, legal assistance, state/federal financial programs, and eligibility).
• Knowledge of medications and their proper administration.
• Proven ability to organize tasks, create action plans, prioritize effectively, and perform under pressure.
• Willingness to work flexible hours and travel locally as needed.
• Maintain current licensure and fulfill mandatory continuing education requirements.
• Fluency in reading, writing, and speaking English.

Additional Information

TB Screening: This position involves direct patient interaction and is part of our Tuberculosis (TB) screening program. If selected, you will be required to undergo TB screening.

Driving Requirements: This role is included in our driver safety program, necessitating a valid state driver's license and personal vehicle liability insurance. You must maintain insurance that meets or exceeds your state's minimum requirements, or $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage, whichever is higher.

The salary range provided below represents a good faith estimate of the starting base pay for full-time employment (40 hours per week) at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.

Salary Range: $65,000 - $88,600 per year
