JOB DESCRIPTION
Company Description
Company is an organization which provides care for individuals with Developmental/Intellectual Disabilities. Our main office is located in Fredericksburg, Virginia, however we have facilities located in Spotsylvania Stafford and Prince William Counties. We are licensed by the State of Virginia and are in compliance with the medical standards of the Department of Behavioral Health and Developmental Services (DBHDS). Our beautiful Single-family homes provide supports and services to individuals with Developmental/Intellectual Disabilities (DD and ID). We provide trainings in Human Rights, TOVA, Medication Management, ID Waiver, First Aid & CPR and much more.
GENERAL:
Under the direct supervision of the Program Director, The Program Manager is responsible for the day-to-day management, administration, and operation of one or more adult community residential group homes serving individuals with intellectual and developmental disabilities (IDD), serious mental illness (SMI), or co-occurring conditions. The Program Manager ensures the delivery of person-centered, high-quality services in accordance with Virginia DBHDS Chapter 105 Regulations, Medicaid Home and Community-Based Services (HCBS) requirements, Human Rights regulations, and organizational policies.
The Program Manager provides leadership and supervision to residential staff, promotes health and safety, ensures regulatory compliance, and supports individuals in achieving maximum independence and community integration.
SPECIFIC:
The PROGRAM MANAGER will:
Administrative:
1. Monitor the group home according to the policies and procedures established by the company Board of Directors and Administration.
2. Demonstrate sound fiscal management of individual accounts in accordance with established company spending guidelines and procedures.
3. Notify the Program Director or Chief Operations Officer of household and maintenance needs.
4. Provide for the safety and security of individuals, staff, and property.
5. Encourage community awareness of the needs and capabilities of people with disabilities and individual programs.
6. Assist with orientation and supervision of volunteers and all staff.
7. Protect the human rights of individuals and safeguard confidentiality.
8. Participate in staff meetings, in-service training sessions, and assist as needed.
9. Establish and maintain appropriate relationships with staff, individuals, families, and other agencies and individuals dealing with the program.
10. Use and further develop personal qualities such as flexibility, common sense, responsibility, problem solving, communication, initiative, and sense of humor.
11. Monitor data and progress on the individuals' training programs, including observation of staff during implementation.
12. Maintain current information on all materials, such as emergency packets, individual records, face sheets, etc.
Programmatic:
1. Act as an appropriate role model.
2. Perform ongoing assessments of the service needs of individuals to include ISPs, quarterly reports and other assessments.
3. Counsel individuals and teach them how to define and solve problems by facilitating self- expression and other means of communication.
4. Encourage integration of individuals into the community.
5. Teach individuals independent living skills, including, but not limited to: money management, food purchase and preparation, personal care, community mobility and
resources awareness, daily program/job responsibilities, use of leisure time and interpersonal skills.
6. Provide transportation, when necessary, of individuals to work sites, appointments, and activities.
7. Advocate human rights and the right to the least restrictive environment.
8. Perform other duties as required.
9. Serve as primary liaison with providers, other agencies, individuals, and related services.
10. Staff and consultants and family members regarding administrative issues; except as stipulated by AP policy or as directed by the AP Director.
11. Promote AP to other agencies; educate and increase awareness on AP activities; participate in and implement assigned activities in the AP marketing plan.
12. Prepare the necessary paperwork, including medication charts, waiver schedules, daily log sheets, etc. for the next month, prior to the end of the month before.
13. Prepare and submit Quarterly Reports, documenting progress of individuals.
14. Assist with and provide input into preparation of Waiver Plans.
15. Assist with writing checks for individuals' accounts to pay bills.
HCBS RIGHTS
In addition to the above responsibilities, the staff will also be responsible to the individual they support the following.
1. Individuals will be integrated in and support full access to the greater community.
2. Ensure an individual's rights of privacy, dignity and respect, and freedom from coercion and restraint.
3. Optimize, but does not regiment, individual initiative, autonomy, and independence in making life choices, including but not limited to, daily activities, physical environment, and with whom to interact.
4. Facilitate individual choice regarding services and support, and who provides them.
Qualifications:
· Minimum three (3) years of experience working with adults with intellectual or developmental disabilities, behavioral health needs, or related populations.
· Minimum two (2) years of supervisory experience in residential or community-based services.
· Experience with HCBS Waiver services preferred.
· Experience with Virginia DBHDS licensing standards preferred.
Must meet AP and funding jurisdiction current requirements for criminal background/records check.
A valid health certificate to meet current funding jurisdiction requirements is required.
Physical Qualifications:
1. Free TB communicable form.
2. Physically able to drive a vehicle and eligible for a driver's license.
3. Able to complete household chores.
4. Able to climb stairs and physically maneuver around the home and community.
5. Able to assist individuals with evaluation.
6. Insurable under Angelic Place's automobile insurance policy.
7. Able to communicate effectively with individuals.
8. Ability to become First Aid and CPR certified.
9. Ability to lift 50 pounds.
