Importance of This Role

As a Home Health Physical Therapist, you play an essential role in the care team, assisting patients in their recovery, helping them regain mobility, and supporting their independence in the environment they feel most at ease: their home.

This position offers a high degree of autonomy and is centered around building relationships, allowing you to witness the tangible effects of your efforts daily.

Your Responsibilities

Deliver Patient-Centered Care

• Assess and evaluate patients’ functional abilities, mobility, strength, and rehabilitation potential.
• Create and execute personalized care plans in collaboration with physicians and the care team.
• Administer hands-on therapy to enhance mobility, strength, balance, and independence.
• Educate patients and their caregivers on treatment protocols, exercises, and safety measures.

Drive Outcomes & Recovery

• Track patient progress and modify treatment plans as necessary.
• Facilitate safe transitions of care and assist in discharge planning.
• Identify additional patient needs and work with interdisciplinary teams.

Collaborate & Communicate

• Work closely with nurses, therapists, and physicians to ensure coordinated care.
• Accurately document patient care and progress in the electronic medical record (EMR).
• Engage in case conferences and team meetings.

Lead & Support

• Oversee Physical Therapist Assistants (PTAs) in compliance with state regulations.
• Provide guidance and training to caregivers and support staff.

Your Qualifications

• Graduation from an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state where you practice.
• 1–2 years of experience (home health experience preferred).
• Strong clinical assessment and critical thinking abilities.
• Excellent communication skills and patient education capabilities.
• Current CPR certification.
• Valid driver’s license, dependable transportation, and auto insurance.
