Travel Hospital Pharmacist - Waukesha, WI

A critical opportunity is available for a travel pharmacist to support a 50-bed hospital located in Waukesha, WI. This role offers a chance to apply your clinical expertise in a dynamic hospital setting, working closely with healthcare teams to ensure optimal patient medication management.

Schedule:

• 12-hour day shifts
• 7 days on, 7 days off rotation

Responsibilities:

• Review medication orders to assess drug therapy appropriateness, including dosing, dosage form, regimen, therapeutic duplication, and potential drug interactions
• Provide pharmacokinetic and anticoagulation dosing guidance
• Deliver pharmaceutical care through clinical services such as medication reconciliation and discharge medication education
• Oversee pharmaceutical compounding activities with a focus on USP regulatory/compliance guidelines (USP 797 and USP 800)
• Utilize EPIC for medication management and documentation

Qualifications:

• Valid pharmacist license in Wisconsin, or the ability to obtain a temporary WI license
• Minimum of 1 year of hospital pharmacy experience
• Strong understanding of hospital pharmacy operations and clinical pharmacy services
• Proficiency with EPIC electronic health records preferred
