Sign-On Bonus: $10,000
This bonus is structured as an earn-as-you-go plan and does not necessitate a contractual employment agreement.
Special consideration will be given to candidates residing in or able to commute to Mecklenburg, Cabarrus, and Union Counties, NC.

As a Home Health RN Case Manager, your duties will include:
- Conducting skilled nursing visits in patients' homes
- Performing assessments, start-of-care visits, and developing care plans
- Collaborating with the interdisciplinary care team
- Supervising LPNs and Home Health Aides
- Documenting care using an iPad with AI assistance (experience with OASIS is a plus)
- Educating patients and their families to foster independence
- Ensuring the continuity, quality, and safety of patient care
- Active and unrestricted Registered Nurse (RN) license
- Valid driver's license and dependable transportation
- Current CPR certification
- Minimum of 1 year of RN experience (preferably in home health or acute care)
- Experience working with aging or adult patient populations
- Strong organizational and time-management skills; ability to multitask, prioritize a busy caseload, and confidently utilize technology and electronic documentation systems
