Physical Therapist PT – Home Health Services - SIGN ON BONUS $15,000 AND RELOCATION $5,000 available
Special consideration will be given to candidates residing in or near Vidalia or Dodge County, Georgia.

JOB PURPOSE:
The role involves providing evaluation, consultation, and treatment interventions for home health patients referred by a licensed physician, while ensuring effective communication with patients, their families, and other relevant stakeholders.
KEY RESPONSIBILITIES:
- Assess the patient's functional level and therapy requirements.
- Administer treatment to alleviate pain, enhance or restore function, and achieve optimal outcomes as per the care plan.
- Consult with families and interdisciplinary team members to support the best functional outcomes.
- Monitor, document, and report on the patient's health progress using required documentation and preferred technology within set timelines.
- Collaborate with the interdisciplinary team and attending physician to ensure continuity of care and manage the patient care plan, making referrals as necessary.
- Exhibit intermediate knowledge of home care and demonstrate competency in specific patient care skills essential for delivering care.
- Conduct field visits to evaluate and initiate care, as well as monitor clinical outcomes and performance improvement initiatives.
- Utilize approved educational materials for patient and family instruction.
- Complete documentation accurately and promptly at the point of care, adhering to industry standards (e.g., OASIS assessments, admissions, recertifications, etc.).
- Meet agency productivity standards while using time and resources effectively.
- Participate in agency surveys, mandatory training, case conferences, staff meetings, and programs as required.
- Follow established safety protocols and report any job-related hazards as needed.
- Maintain current licensure, certification, and clinical records to comply with federal, state, and agency regulations.
- Recognize and respond to patient care priorities, demonstrating initiative and sound judgment.
- Adapt to changes in workload and assist in training or orienting new team members.
- Take responsibility for reporting any suspected violations to supervisors until resolved.
- Demonstrate self-direction, effective time management, and the ability to meet strategic goals by deadlines.

LICENSURE, CERTIFICATION AND EDUCATION REQUIREMENTS:
- A valid professional license in the state of practice is required.
- Graduation from an accredited Physical Therapy Program is necessary.
- If trained abroad, the program must be recognized as substantially equivalent to U.S. entry-level education by an APTA-approved credential evaluation organization.
PREFERRED SKILLS AND EXPERIENCE:
- Experience in Home Health is preferred.
