Overview:
\n Our organization is seeking a Wound Care Nurse Practitioner to provide specialized wound management within skilled nursing facilities in Pittsburgh. This role focuses on comprehensive wound assessment, individualized treatment planning, and interdisciplinary care coordination to directly improve patient outcomes.

Position Offers:

  • Schedule: Tuesday, Wednesday, Thursday rounding, flexible

Key Responsibilities:

\n
    \n
  • Perform comprehensive wound assessments, including etiology identification, staging, and risk stratification
  • \n
  • Develop and implement individualized wound treatment plans, including dressing selection, debridement, and adjunctive therapies
  • \n
  • Manage wound-related infections through timely recognition and collaboration with prescribing providers
  • \n
  • Lead pressure injury prevention initiatives, including risk assessment and staff education
  • \n
  • Provide specialized management for diabetic foot ulcers, including offloading and vascular coordination
  • \n
  • Coordinate interdisciplinary care with physicians, nursing staff, therapists, and dietitians
  • \n
  • Document assessments and care plans in the EHR to support quality improvement
  • \n
  • Educate patients and caregivers on wound self-care, dressing changes, and infection prevention
  • \n
\n

Qualifications:

\n
    \n
  • Active NP or PA licensure in Pennsylvania
  • \n
  • National board certification as a Nurse Practitioner (ANP, FNP, or AGNP)
  • \n
  • Proven clinical experience in wound care management within post-acute or long-term care settings
  • \n
  • Current BLS certification; ACLS preferred
  • \n
  • Formal wound care certification preferred
  • \n
