What You’ll Do
Deliver Patient-Centered Care
- Assess and evaluate patients’ functional abilities, mobility, strength, and rehabilitation potential
- Create and execute personalized care plans in collaboration with physicians and the care team
- Provide hands-on therapy to enhance mobility, strength, balance, and independence
- Educate patients and caregivers on treatment plans, exercises, and safety measures
Drive Outcomes & Recovery
- Track patient progress and modify treatment plans as necessary
- Facilitate safe transitions of care and assist in discharge planning
- Identify additional patient needs and work with interdisciplinary teams
Collaborate & Communicate
- Work closely with nurses, therapists, and physicians to ensure coordinated care
- Accurately document patient care and progress in the EMR
- Participate in case discussions and team meetings
Lead & Support
- Oversee Physical Therapist Assistants (PTAs) in accordance with state regulations
- Provide guidance and education to caregivers and support staff
What You Bring
- Graduate of an accredited Physical Therapy program (DPT or Master’s)
- Current PT license in the state of practice
- 1–2 years of experience (home health experience preferred)
- Strong clinical assessment and critical thinking abilities
- Excellent communication and patient education skills
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
