As a Home Health Physical Therapist, you play an essential role in the care team, aiding patients in their recovery, regaining mobility, and maintaining independence in their preferred environment: home.
This position offers a high degree of autonomy and is relationship-focused, allowing you to witness the tangible effects of your efforts daily.
Your ResponsibilitiesDeliver Patient-Centered Care
- Assess and evaluate patients’ functional abilities, mobility, strength, and rehabilitation potential.
- Create and execute personalized care plans in collaboration with physicians and the care team.
- Administer hands-on therapy to enhance mobility, strength, balance, and independence.
- Educate patients and caregivers on treatment protocols, exercises, and safety measures.
- Track patient progress and modify treatment plans as necessary.
- Facilitate safe transitions of care and assist in discharge planning.
- Identify additional patient needs and work with interdisciplinary teams.
- Work closely with nurses, therapists, and physicians to ensure coordinated care.
- Accurately document patient care and progress in the electronic medical record (EMR).
- Engage in case conferences and team meetings.
- Oversee Physical Therapist Assistants (PTAs) in accordance with state regulations.
- Provide guidance and education to caregivers and support staff.
Qualifications
- Graduate from an accredited Physical Therapy program (DPT or Master’s).
- Current PT license in the state of practice.
- 1–2 years of experience preferred (home health experience is a plus).
- Strong clinical assessment and critical thinking abilities.
- Excellent communication and patient education skills.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and auto insurance.
