Per Diem Physician - Post Acute & Senior Care Services (Lititz)
Job Description
Our organization is on the lookout for Per Diem physicians specializing in adult and senior care to enhance our expanding post-acute care team in the Lancaster/Lititz area. This role is vital in delivering exceptional, compassionate care to our elderly patients. The post-acute care team operates across various skilled nursing facilities throughout the county.
We encourage applications from primary care physicians, hospitalists, and those with geriatric training.
- Deliver direct patient care across multiple settings within the aging spectrum, including skilled nursing, rehabilitation, personal care/assisted living, independent living, and clinics.
- Engage with facility leadership, hospital discharge teams, care management, and various specialties to ensure high-quality care aligned with shared objectives.
- Post-acute care encompasses a diverse range of healthcare fields, from general primary care and internal medicine to geriatrics and acute rehabilitation services. Our clinicians collaborate with various specialists, enhancing their knowledge and skill sets.
- Work in partnership with and oversee Advanced Practice Providers.
- Utilize EPIC EMR, along with online diagnostics such as PACS imaging and lab results.
- Participate in clinical rounds Monday through Friday, with appealing daily schedules and flexible models. Overnight and weekend on-call duties are limited to phone coverage, shared equitably among partners and APPs, with favorable ratios.

Professional Qualifications:
- Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.

- Eligibility for licensure in Pennsylvania.
