In South West London our Clinical Assessment Service plays a vital role in supporting patients across the regionproviding timely, expert clinical advice and directing them to the most appropriate care pathway.

• As a General Practitioner, youll be part of a dynamic, multidisciplinary team working within a telephone and digital triage environment.
\n
• Youll help ensure patients receive the right care, in the right place, at the right timereducing unnecessary hospital visits and improving outcomes across the system.
\n

• Delivering remote clinical assessments via telephone and digital platforms for patients with urgent and unscheduled care needs.
\n
• Making safe, evidence-based decisions to direct patients to appropriate services, including self-care, primary care, urgent treatment centres, or emergency services.
\n
• Applying advanced clinical knowledge to assess undifferentiated presentations and manage risk effectively.
\n
• Supporting clinical governance, contributing to service development, and mentoring colleagues.
\n
• Collaborating with GPs, paramedics, mental health professionals, and other clinicians to ensure integrated care across the region.
\n

• Fully qualified General Practitioner with GMC registration and relevant UK licence to practice.
\n
• Previous experience working in an Out of Hours setting is essential.
\n
• Proven experience using the Adastra clinical system for patient record management.
\n
• Strong clinical assessment skills and the ability to manage a diverse and often complex caseload.
\n
• Excellent communication and interpersonal skills with the ability to work both independently and as part of a team.
