Primary Care

We are seeking a dedicated healthcare professional to provide medical services to Veterans, primarily in an outpatient primary care environment. The role involves managing various health conditions, including but not limited to diabetes, hypertension, hyperlipidemia, chronic obstructive pulmonary disease, and mental health disorders, with a strong emphasis on preventive care.

• Lead the Patient Aligned Care Team (PACT) model, promoting a collaborative approach to healthcare delivery.
• Accurately document all medical information using electronic medical records.
• Conduct medication reconciliation to ensure patient safety and effective treatment.
• Deliver high-quality outpatient care while adhering to compliance standards.
• Maintain up-to-date clinical privileges, licensure, and certifications, including Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS).
• Engage in medical duties related to quality assurance, peer review, and performance improvement initiatives as directed by the Chief of Medical Service.
• Perform any additional tasks as assigned.

• Contract Buy Out Program: Potential eligibility for the Physician/Provider Contract Buy Out Program, allowing for the buyout of service contracts in exchange for employment at designated rural or highly rural facilities, with a minimum service obligation of four years.
